This chapel is located on the site of the prison where St. John Sarkander was tortured to death in the 1600s. He was accused by Protestants of providing info for the invasion of a Catholic army from Poland. In the basement, there is a rack (of the torture-device kind) from the time period (evidently not the same one on which he died).

From the upper square (Horni Namesti), walk northeast on Ztracena, turn right on Michalska, then almost immediately left on Mahlerova. The chapel is at the end of "block"(not really a block) where Mahlerova and Na Hrade meet.
